Notes feature not working properly

The notes feature is not giving me a complete breakdown of the document I upload. Previously, it used to work without any issues but since the recent updates, I am having issues with this feature. The issue is that it gives me about half of the notes on a document instead of a full/complete note breakdown from the document uploaded.
